We are using a new online service called Register with a GP Surgery that makes it easy to register with a GP Surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one. You can download the form here.
If you are new to the area and are requiring to register with one of our GP’s please ask at our Reception. Please bring along with you Photo ID and proof of address. Prior to your registration you will be required to have a new patient check to enable your registration to be fully complete. An appointment will be offered with our Health Care assistant during your registration.
We encourage all patients to register at a surgery local to their home address, if however you wish to register as an out of area patient please print off, complete, sign and return the letter with your completed registration form. The surgery will then review your out of area registration request and decide whether to accept you which depends on a number of factors regarding your health requirements.
